# One Page Case — Document Gathering Checklist

Use this as a preparation checklist, not a universal legal discovery list. Requirements vary by jurisdiction and case. Ask your attorney which documents are actually required and what date range to use.

## Protect sensitive information

- Keep originals in a secure location.
- Redact Social Security numbers and full account numbers before sharing when appropriate.
- Do not paste passwords, private keys, authentication codes, or children's sensitive identifiers into an LLM.
- If you use an LLM with this checklist, instruct it to label unknowns and never invent, infer, or "fill in" a missing fact, amount, date, document, or legal requirement.
- Record where a document came from and its statement/valuation date.

## Case and deadlines

- [ ] Petition, response, temporary orders, prior agreements, and court notices
- [ ] Mediation, hearing, disclosure, and response deadlines
- [ ] Existing parenting plan, support worksheet/order, or protection/restricted-contact order

## Income and recurring costs

- [ ] Recent pay statements and the most recent tax returns requested by counsel
- [ ] Bonus, commission, self-employment, unemployment, pension, or benefit records
- [ ] Health-insurance premiums and childcare invoices
- [ ] Existing child-support and alimony orders plus payment history

## Parenting and transportation

- [ ] Current two-week schedule and actual deviations
- [ ] School and childcare calendars
- [ ] Exchange locations, transportation pattern, and recurring activity schedule
- [ ] Holiday, break, travel, and communication arrangements

## Assets

- [ ] Real-estate title, current valuation/appraisal, mortgage statement, and repair estimates
- [ ] Bank statements with owner/title and statement date
- [ ] Retirement and investment statements with valuation date
- [ ] Crypto wallet/exchange statements with a consistent valuation date; never share private keys
- [ ] Vehicle title, loan statement, and valuation source
- [ ] Business-interest records and any professional valuation
- [ ] Separate-property source documents if a tracing claim may exist

## Debts

- [ ] Current statement, original/current balance, interest rate, and minimum payment
- [ ] Account holder(s), authorized users, and date incurred
- [ ] A short description of what expenses are included
- [ ] Tax, medical, student, personal, and family-loan documentation

## Reimbursements, offsets, and arrears

- [ ] Post-separation payments with date, amount, payer, purpose, and proof
- [ ] Repairs, insurance, childcare, loan paydowns, and sale losses
- [ ] Arrears calculation and source records
- [ ] Double-count check: confirm an expense is not claimed separately if it already sits inside a debt balance

## For every important number

Record:

## Before the attorney meeting

- [ ] Put the five largest financial items on one page
- [ ] Highlight every missing, estimated, or disputed number
- [ ] List the next three deadlines
- [ ] List the five decisions you need help making
- [ ] Ask which documents are unnecessary so you do not pay to organize noise
